The Medical Records Department ensures and maintains the highest level of confidentiality and security of personal and health information contained in medical records. The team also ensures prompt, efficient and courteous service to all areas, health professionals, and clientele who utilize KMHC’s services.
Mission: To provide accurate and holistic health information services to enable quality patient care.
Activities & Services:
- Coding and abstracting of clinical data from the health records of discharged inpatients. Qualitative and quantitative data analysis, government discharge reporting as well as statistical reporting.
- Registering new patient and opening a new medical file
- Managing outpatients’ incoming documents by mail/fax for physicians and health professionals
- Ensuring conservation of inpatients’ & outpatients’ medical records on both paper mode and electronic mode
- Medical transcription
- Processing requests for medical information respecting the laws and regulations in force
Access to medical records
To obtain a copy of your medical records, please complete and sign the Authorization form, and follow the User guide. For other information, such as parental consent, access to a deceased patient’s file, and anything regarding the release of health information, please refer to the “User Guide/MRD Procedure on Obtaining a Copy of Medical Records”.
